Please come in 15 minutes early prepared to fill out paperwork or have the exam form pre filled out. Make sure you have a list of all medical conditions with dates and medications with dosage and frequency. Arrive ready to give a urine sample. If you have any of the following medical conditions additional paperwork is needed from your treating physician. You may need additional paperwork for other medical conditions but these are the most common ones requiring additional paperwork.
CONDITIONS REQUIRING ADDITIONAL PAPERWORK
Diabetes

HgA1c ( within last 3 mo)

Diabetes form from treating physician if insulin dependent diaabetes (filled out in last 45 days)
Sleep Apnea

CPAP usage report for the last 3 months
Heart Conditions ( Heart Attack/ stent)

Clearance letter from cardiologist

Exercise tolerance test

Defibrillator are an automatic disqualifier
Medications

Medications that may cause sedation or sleepiness or controlled substances (includes narcotics, sleeping pills, anxiety medication, ADHD medication)

You will most likely need a note and medical records from your treating physician regarding the safety of driving a DOT vehicle while using these medications. You can bring in Medication form filled out by prescribing physician
Medical Marijuana is not federally legal even if you have a Medical Marijuana Prescription it is disqualifying for DOT
